Journey Forward

In its 2005 report, From Cancer Patient to Cancer Survivor: Lost in Transition, the Institute of Medicine revealed poor cancer care coordination and a deep lack of awareness about survivorship needs among cancer survivors.1 These issues can affect cancer survivors' long-term physical and psychosocial health.

Journey Forward offers tools and resources for patients and healthcare providers to build a cancer survivorship care plan. A survivorship care plan is:

  • A coordinated post-cancer treatment plan
  • For the cancer survivor, oncology team, primary care physician, and other healthcare providers
  • Created by the oncology provider and survivor in partnership
